Measurements should be professionally taken. They should measure bust, waist, and hip
at the fullest area in order to prevent tightness.

Refer to size charts given in the store or for phone orders, one can be faxed or emailed to
you. IMPORTANT: Please remember that manufacturers suggest ordering a size based
on your largest measurement, however the size chosen is ultimately your decision!

This form along with a 50% deposit is required before any order will be placed. Phone
orders may fax or mail the contract back to The White Room.
